Sports Physicals

Physical Information

All students wishing to participate in Middle School Sports tryouts must have had a physical within the past calendar year.  Any physical dated past May 1 will be good through the end of the NEXT school year.

For instance, if a student had a physical on March 18, 2013 he/she would be able to play interscholastic sports through March 17, 2014.  If they had a physical on May 12, 2013 they would be able to participate through the end of June, 2014.

Additionally, your student must turn in a signed copy of the “Athletic Participation Agreement.”  If an eighth grader intends to play a sport at the high school level they must also have sports injury or medical insurance.

Physicians and parents must fill out these forms.  All sections, including the “Athletic Participation Agreement” must be completed.  Return forms to Mr. Siegel.

Insurance Information

Students have the option of purchasing, at a relatively cheap price, insurance from the provider below.  All students playing high-school sports must have insurance, it is strongly encouraged for middle school.  Coverage options start at $15.00.  APS and Jefferson Middle School are not financially responsible for injuries that occur during interscholastic sports or other school activities.
