Sports Physicals/Accident Insurance

Physical Information

All students wishing to participate in Middle School Sports tryouts must have had a physical within the past 14 months.

For instance, if a student had a physical on Aug 1, 2024 he/she would be able to play interscholastic sports through Oct 1, 2025.

Physicians and parents must fill out these forms.  All sections, including the “Athletic Participation Agreement” must be completed.  Return forms to Mr. Siegel or drop forms off in the front office and ask to have them put into Mr. Siegel’s mailbox.

Currently, sports physicals are available on a walk-in basis at INOVA Urgent Care Centers for $25.00.  Take advantage!

Discount Priced Accident Insurance

If your son or daughter is injured while participating in sports or activities at Jefferson Middle School your family is responsible for any medical costs which may be incurred.  For instance, if during PE your child sprains their ankle and requires a visit to a doctor or hospital, your family is responsible for handling those bills.

Arlington Public Schools provides access to discount priced accident insurance to cover injuries to your child.  Options are available to cover your child’s medical bills if something happens while they are at school or (for an additional charge) anytime day or night during the school year.  Follow the links below for additional information about accident insurance options.
